Social Workers (SWs) are highly skilled health professionals who specialize in addressing a wide range of social issues, including but not limited to oppression, discrimination, domestic violence, unemployment, and poverty. Collaborating closely with you and your family physician, the Social Worker aims to enhance both individual and collective well-being.

What happens at my appointments?

Our PCN Social Worker is available to meet with you individually to discuss your needs and create a personalized plan of care that aligns with your goals. They can also provide guidance in accessing additional support services, if needed.

You can access this services by asking for a referral from your Wetaskiwin PCN family physician. This service is by appointment only.
