What is a Health Navigator?
The primary role of the Health Navigator is to encourage patient self-determination, providing information to make healthcare choices/or receive medical care with an enhanced sense of confidence about risks, benefits and responsibilities.
Personalized guidance to help move through the system by empowering with knowledge. Providing referrals to Allied Health Professionals in the Wetaskiwin PCN with connected Doctors from community. Provide referrals for medical follow-up of patients with no family physician.

What happens at my appointments?
Our PCN Health Navigator is available to meet with you individually to discuss your needs and create a personalized plan of care that aligns with your goals. They can also provide guidance in accessing additional support services, if needed.

Services which can be included:

  • Intake to look at all areas of patient needs.
  • Medical/Mental Health/Social needs assessment
  • Provide referrals as required to Allied Health Professionals in Wetaskiwin PCN.
  • Guidance, phone numbers, websites to connect with other Agencies.
  • Advocate for client if needed to other agencies.
  • Connect patients with appropriate cultural support, in a wholistic approach. While providing a safe space to connect.
  • If needed you can have family present to discuss issues and assist the patient.

How do I meet with the Health Navigator?
You can access this services by doing a walk-in or calling the PCN and booking an appointment.
